Risk Management

Claims Administration

The Risk Management Division is responsible for oversight of claims administration, including the City’s worker’s compensation program; administration of insurance contracts; and loss prevention for City employees and citizens.

If you believe you have incurred an injury or damage to property caused by the City of Prescott, you may file a claim with the City Clerk’s office.   

To obtain the claim form please contact the Risk Division at 928-777-1274.  

Arizona Revised Statute sets forth the requirements to file a valid notice of claim against the City. 

It is recommended, that should you wish to file a claim, that you seek independent legal advice.
